In addition to the forms listed above, each contestant MUST provide a copy of their birth certificate, talent CD and college transcript.

Each contestant must also send in a photograph (head shot) for our program book.

EVERYTHING MUST BE SUBMITTED TO THE MISS MIAMI OFFICE BY MARCH 1, 2010.
